Recovery timeline checklist
General milestones aligned with common hospital discharge education and orthopedic rehab phases. Your surgeon's written protocol always takes priority.
Before surgery
- A medication manager app (e.g., MediSafe) helps when juggling pain meds and other prescriptions early on.
- Complete pre-hab (range of motion and quad activation) if prescribed. Stronger knees before surgery often recover faster.
- Understand weight-bearing instructions (touch-down vs partial vs full) for your graft type and any meniscus work.
- Order crutches, ice packs or cryotherapy, and knee brace only if included on your surgeon's prescription.
- Clear space at home for crutch paths; plan time off work/school for the first week and limited driving initially.
- Stock easy meals and hydration; arrange rides to follow-up appointments.
Week 1 (protection phase)
- Wear knee brace and use crutches per protocol; do not bear more weight than allowed.
- Elevate the leg above heart level several times daily; ice 20 minutes on/off unless using a continuous cold device.
- Keep dressings dry unless told otherwise; watch for calf swelling or shortness of breath (blood clot warning signs).
- Do not place pillows or rolled towels under the knee. Keeping the knee straight (extension) is an early priority after ACL surgery.
- Begin ankle pumps and quad sets immediately if cleared; they help circulation and prevent stiffness.
- Avoid bending the knee beyond limits set by your surgeon (often restricted first 1–2 weeks with some protocols).
- Take pain medication before PT exercises if directed; swelling peaks around days 3–5 for many patients.
Weeks 2–6
- Gradually increase allowed knee flexion per protocol; attend all PT sessions.
- Wean crutches when you can walk without a limp and your surgeon/PT agrees.
- Brace use may taper. Follow written orders, not social media timelines.
- Stationary bike with no resistance is common once you have sufficient bend.
- No running, jumping, or cutting until much later in rehab.
Months 3–9+ (return to sport pipeline)
- Progress through strength, balance, and plyometric phases with sports PT testing.
- Return to sport only after passing hop tests and surgeon clearance. This is often 9–12+ months for cutting sports.
- Continue home strength program to reduce re-injury risk.
- Mentally plan for setbacks. Swelling after busy days can occur months in; communicate with your PT.
