Recovery timeline checklist
General milestones aligned with common hospital discharge education and orthopedic rehab phases. Your surgeon's written protocol always takes priority.
Before surgery
- Optimize nicotine cessation and nutrition for bone healing.
- Fit the brace you will use postoperatively if ordered.
- Set up bathroom equipment and a firm mattress/chair strategy.
- Plan extended time off for physical jobs.
Weeks 0–2
- Brace on per protocol; log-roll always.
- Short walks; ice; incision care.
- Strict lifting caps (often 5–10 lb).
- No bending/twisting; no baths until cleared.
Weeks 2–8
- Walking endurance grows; PT starts when cleared.
- Driving when brace rules, narcotics, and control allow.
- Desk work may return with frequent position changes.
- Imaging follow-up monitors early hardware position.
Months 3–12
- Bone fusion consolidates over months. Activity advances in stages.
- Strengthening under guidance; avoid smoking relapse.
- Higher-impact sport/work needs formal clearance.
- Report new neurologic deficits or wound issues anytime.

Radiographic fusion often takes many months. Feeling better at 6 weeks is not full fusion.
Why is smoking such a big deal?▾
Nicotine impairs bone healing and raises nonunion risk after fusion.
Is this harder than microdiscectomy?▾
Usually yes. Longer BLT limits, possible bracing, and slower return to lifting.
When can I bend to tie shoes?▾
Only when your surgeon advances flexion precautions. Use slip-ons early.
